RESEARCH ARTICLE
Decrease in dysbaric osteonecrosis severity as a result of 45-minute oxygen pre-breathe
Jacob Lamers; Kent MacLaughlin; Dave Pegelow; Marlowe Eldridge, MD; Aleksey Sobakin DVM, PhD
Objectives:
- Understand that using a 15-minute vs a 10-minute compression profile will reduce the observed number of patients requiring a stop due to Eustachian tube dysfunction, during a compression to 45 fsw in a multiplace setting.
- Understand that reducing the number of stops and holds (symptomatic Eustachian tube dysfunction), should result in less barotrauma and less of a tendency to terminate a patient’s treatment.
- Understand that a linear compression profile has an observed effect on reducing Eustachian tube dysfunction over a non-linear compression profile, however this finding is not statistically significant and is pending further study in the Phase III arm.
- The protocol studied in the multiplace setting may prove useful during compression in a monoplace chamber, but this protocol has not yet been studied in a monoplace setting.
- To understand the effect of shorter lengths of oxygen pre-breathe in the reduction of dysbaric osteonecrosis (DON) and recommendations to prevent DON.
- To explain how the main (multiple exposure to radical drop in pressure, greater maximal diving depth) and other potential (higher level of plasminogen activator, fat embolisms, hyperlipidemia) risk factors contributed to the development of DON.

Goal
For over 45 years the UHMS has been the primary source of information for hyperbaric medicine and physiology worldwide. This evidence based, peer-reviewed scientific journal-based CME has as its goal to address identified professional practice gaps and provide a quality CME opportunity which will improve scientific knowledge and skills for undersea, hyperbaric and wound care physicians, nurses and technicians, and other clinicians and personnel whose practice includes undersea, hyperbaric medicine and wound care.
Target Audience – Included in the “goal”
Date of the activity’s original release – August 31, 2022
Termination date – August 31, 2025
Accreditation Statement: The Undersea and Hyperbaric Medical Society is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
Designation Statements:
Physician CME: The Undersea & Hyperbaric Medical Society designates this journal-based activity for a maximum of 2 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nursing/RRT Contact Hours: 2
License types: RN, LPN, CNS, ARNP, CAN. Provided through the Florida State Board of Nursing and the CE is reciprocal for nurses in the United States. Florida nursing credits are reciprocal and approved for nurses within all states. Receiving credit for Florida providers is simple, attend the course and our staff will upload your credits directly to the Florida State database. For out of state credit, we provide all the necessary paperwork for you to file with the respective nursing board.
NBDHMT Category A: This journal-based activity is approved for 2 Category A credit hours by National Board of Diving and Hyperbaric Medical Technology, 9 Medical Park, Suite 440, Columbia, South Carolina 29203.
- NBDHMT Accreditation Statement: For CHT recertification purposes, the NBDHMT requires a minimum of nine of the minimum 12 required Category A credits relate directly to any combination of hyperbaric operations, related technical aspects and chamber safety.
